AFFIDAVIT OF COLLECTION AND DISTRIBUTION OF
DECEDENT'S PERSONAL PROPERTY
The undersigned, being first duly sworn, on oath depose and
state that we are making this affidavit pursuant to Section
2-1-201, W.S.A. (2005), on behalf of ourselves as distributees, as
hereinafter set forth, and that we make the following statements in
connection therewith:
1. That Ted L. Roberts died on February 17, 1999, in
Kemmerer, Wyoming, and \,,¡as a resident of Lincoln County,
State of Wyoming, at the time of his death; that said
decedent died intestate; that we are the sole and only
parties entitled to the property of the decedent pursuant
to the laws of the State of Wyoming.
2. That Annie Roberts, a/k/a Ann Roberts, died on July 24,
2005, in Salt Lake City, Utah, and was a resident of
Lincoln County, State of Wyoming, at the time of her
death; that said decedent died intestate; that we are the
sole and only parties entitled to the property of the
decedent pursuant to the laws of the State of Wyoming.

That the value of the entire estate
administration wherever located, less
encumbrances, does not exceed $150,000.00.

date of death of the decedents.
5. That no application or petition for the appointment of a
personal representative of said decedents is pending or
has been granted in any jurisdiction.
6. That the onl y personal property owned solely by said

(b) 1988 GMC (VIN 1GTFK24K5JZ543966);
(c) 1969 Jet Travel Trailer (Serial No. 17-69-3232)
(d) 1976 Pende Mobile Home (VIN S6963)

7. The undersigned request that the titles to said vehicles
be transferred as follows:

Roberts;
to John P.
(c) 1969 Jet Travel Trailer (Serial No. 17-69-3232)
to John P. Roberts; and
(d) 1976 Pende Mobile Home (VIN S6963) to John P.
Roberts, Kathleen A. Wakefield, Bonnie M.
Weston, Laurie J. Miller, and Fabian Gene
Malatare.
8. The undersigned request that the remaining balances in
the two (2)ING accounts be paid to John P. Roberts,
Kathleen A. Wakefield, Bonnie M. Weston, Laurie J.
Miller, and Fabian Gene Malatare, in equal shares.
