COMES NOW, the Evans Family Revocable Living Trust, ult/a dated 9 April, 1993 acting
through its Trustees, Monty A. Evans and Sherry J. Evans, of Teton County, Wyoming, who,
being duly sworn on oath according to law, do hereby make the following statements of facts and
affinn:
1. That the Evans Family Revocable Living Trust holds a Mortgage dated 31 January 2001 and
recorded 25 March 2005 in the Office of the Clerk of Lincoln County in Book 581 of
Photostatic Records on page 442, for part of the property described under the Certificate of
Surveyor on the plat titled, "ELK RIDGE RANCHES AT HENRY'S MOUNTAIN WITHIN
THE N1I2 SECTION 24 T35N Rl19W LINCOLN COUNTY, WYOMING", dated 14
September 2005, as revised.
2. That the undersigned have reviewed said plat.

CJ Mortgagee thereon and the implications thereof.
4. That this affidavit is signed in the stead of and has the same effect as if the original mylar of
said plat had been signed.
