1. That she knows of her own personal knowledge that S. Reed Dayton, the decedent
mentioned in the attached Certified Copy of Certificate of Death is one and the same person as S.
Reed Dayton who is named as one of the original co-trustees of the S. Reed and Lois Dayton Family
Trust dated March 19, 1993.
2. That she knows of her own personal knowledge that Lois T. Dayton, the signer of the
attached Resignation of Trustee is one and the same person as Lois T. Dayton who is named as one
of the original co-trustees of the S. Reed and Lois Dayton Family Trust dated March 19, 1993.
3. That Barbara D. Perry and Sharon R. Dayton are appointed as successor co-trustees of
the S. Reed and Lois Dayton Family Trust dated March 19, 1993 pursuant to he provisions of Article
XX of said Trust.
