AFFIDA VIT OF SURVIVORSHIP
We, Kelly and Debra Heiner, being first duly sworn state the following:
1. That several years ago, we purchased the property described in Attachment A,
from Charles Delbert Heiner.
2. That the correct deed transferring the property into our names was never filed
before the death of Charles Delbert Heiner.
3. That Charles Delbert Heiner died on June 30, 1999 as evidenced by the attached
Death Certificate identified as Attachment B.
4. That all other heirs having a claim to the property have conveyed their interest in
said property to Kelly and Debra Heimer (See Lincoln County Clerk Records, Book 483 PR Page
337 through Book 483 PR Page 354.
. 5. That we are the sole owners of said property and tax notices and other county
documents should reflect such.
FURTHER the affiants sayeth not.
