1. This form must be cmnpleled and submitted for m~y assignment of Slate Oil & Gas Lease leasehold/merest, tmdivided or di'&P,d as to separate tracts, formations, zones or deposits, and for overriding royalty
imerest.
2. Assignmeots of less than a 100% leasehold interest, do not relieve the assignor of the obligations as a lessee trader the tenffs of tile lease regarding the interest conveyed unless tire State first bas in hand,

3. A signed original and one copy of each assignment must be filed with tile Wyoming State Land and Farm Loan Office. ~"
4. Each assignnrent, if approved, is recognized by the State as effective as of the date of approval by the Director. Adequate b~nd inest be fomisbed covering any and all producti~:e zones prior to approval

8. Co~poratio6s limited liability co npanles and li~nited partnerships must be qualified with the Wyoming Secretary of States O?ice and general partnership papers are reqoired to be filed with the State Land
and Fatal Loan Office prior to, and as a conditinn of Board approval of any assigmnent.
9. Assibmments will ~mt be considered for approval where assignor or any assignee currently has a delinqueot (late) royalty c~' ~ental payment obligation(s) for any State sobsurface resonrce lease.
