AFFIDAVIT OF SURVIVORSHIP
I, ELAINE P . SLATER, a resident of Thayne, Lincoln, Wyoming, residing at
Lincoln County, Wyoming, being of legal age and fIrst duly sworn on oath according to
law, depose and say:
That I am the surviving wife of JAMES ROY SLATER, who died on July 19,
1993 at Thayne, Lincoln, Wyoming, a copy of the official death certificate being
annexed hereto as Exhibit" A";
That your affiant and her deceased husband were the owners of certain real property in
Lincoln County, Wyoming, vested in James Roy Slater and Elaine P. Slater, husband
and ~fe, as joint tenants with the rights of survivorship";
That attached hereto as Exhibit "B" is a description of the property situate in Lincoln
County, Wyoming, so owned.

/ I do further certify under oath that the deceased, JAMES ROY SLATER, was the same
J party named in and whose death terminated his estate in the above-described real

This Affidavit is made for the purpose of complying with Section 34-6-102 and 34-6-
103, Wyoming Statutes, 1977, providing for the termination of joint tenancies in the
appropriate statutory manner.

QUITCLAIM DEED
KNOW ALL MEN BY THESE PRESENTS, THAT ELAINE P. SLATER. a single women. grantor of
County of Lincoln, and State of Wyoming, for and in consideration ofTen Dollars ($10.00) and other
valuable consideration, in hand paid, receipt of which is hereby acknowledged, have hereby remised,
released and forever quitclaimed unto Allen Deland Lainhart and Olivia D. Lainhart, husband and wife,
grantees, all such right, title, interest. property, possession, claim and demand, as she may have or ought to
have, in or to all the following described property, to-wit:
