COMES NOW, 1st Bank, aIkIa First National Bank. Alpine Branch whose address is P.O. Box
3110 Alpine, Wyoming 83128, acting through its undersigned officer, being duly sworn on oath
according to law, does hereby make the following statements of facts and affirms:
1. That 1st Bank, aIkIa First National Bank. Alpine Branch holds a Mortgage recorded in the
Office of the Clerk of Lincoln County in Book 621 of Photostatic Records on page 33, for
all of the property described under the Certificate of Surveyor on the plat titled,
"CLEAR VIEW VILLAGE PHASE I WITHIN THE NEY4SEY4 SECTION 10 T35N R119W
LINCOLN COUNTY, WYOMING" dated 18 November 2005, as revised.
2. That on behalf of 1st Bank aIkIa First National Bank, Alpine Branch the undersigned has
reviewed said plat.
3. That the undersigned has read and understands the provIsIons of the Certificate of
Mortgagee thereon and the implications thereof.
4. That this affidavit is signed in the stead of and has the same effect as if the original mylar of
said plat had been signed.
