COMES NOW, the Patricia R. Graf Moss Trust, dated 11 September 1996, acting
through it's Trustees, Robert H. Moss and Patricia R. Moss whose address is 1685 West
1400 North, Provo, Utah 84604 who, being duly sworn on oath according to law, do
hereby make the following statements of facts and affinn:
1. That the Trust is the owner of Lot 808 and Lot 810 of Lakeview Estates Third
Addition to the Town of Alpine of record in the Office of the Clerk of Lincoln
County, Wyoming as Plat No. 250-D;
2. That the undersigned have reviewed the plat titled, "LAKEVIEW ESTATES
TENTH ADDITION BEING PART OF LAKEVIEW ESTATES INCORPORATED
TRACT (A) WITHIN THE INCORPORATED LIMITS OF THE TOWN OF
ALPINE WITHIN S~SEIj.¡ SECTION 29 T37N R118W LINCOLN COUNTY,
WYOMING", dated 7 October 2004, as revised, which includes the above described
land;
3. That the undersigned have read the Certificate of Owner on said Tenth Addition plat
and understand the provisions and the implications thereof;
4. That this affidavit is signed in the stead of and has the same effect as if the original
mylar of said plat had been signed.
