COMES NOW, Jackson State Bank and Trust whose address is 50 Buffalo Way, Jackson,
Wyoming, 83001 acting through its undersigned officer, being duly sworn on oath according to
law, does hereby make the following statements of facts and affirms:
1. That Jackson State Bank and Trust holds a Mortgage dated 12 January 2004 and recorded 23
January 2004 in the Office of the Clerk of Lincoln County in Book 546 of Photostatic
Recrods on page 6146 and a Mortgage dated 27 January 2006 and recorded 13 February
2006 in said Office in Book 612 of Photostatic Records on page 155, for aportion of the
property described under the Certificate of Surveyor on the plat titled, "DOUBLE L
RANCH WEST SUBDIVISION WITHIN GLO LOT 4 SECTION 21 GLO LOT 1 GLO
LOT 2 GLO LOT 3 W~NE% NY2Sm~ SECTION 28 T36N R119W LINCOLN COUNTY,
WYOMING", dated 25 May 2006, as revised.
2. That the undersigned has reviewed said plat.
3. That the undersigned has read and understands the provisions of the Certificate of
Mortgagee thereon and the implications thereof.
4. That this affidavit is signed in the stead of and has the same effect as if the original mylar of
said plat had been signed.
