AFFIDAVIT OF SURVIVORSHIP

PATTY CERRONE, being first duly sworn upon her oath, deposes and states as follows:
1. I am the natural mother of Erick Daniel Cerrone.
2. On or about the 28th day of February, 2006, my son, Erick Daniel Cerrone, died as is
evidenced by the official certificate of death attached hereto and incorporated herein by this
reference.
3. On or about the 20th day of May, 2006, my son, Erick Daniel Cerrone's, body was
found as is evidenced by the official certificate of death attached hereto and incorporated herein
by this reference.
4. At the time of his death, he was the sole account holder for checking account no.
414023002 at First National Bank, now known as First Bank.
5. By reason of my son, Erick Daniel Cerrone's death, I am entitled to sole ownership
and title of the above referenced bank account.
