AFFIDAVIT OF CORRECTION
To Correct the subdivision plat of the IBt Amended Plat of Cottonwood Creek Subdivision Phase I,
A Simple Subdivision, recorded July 21, 2006 as instrument no. 920465.
I, Kim H. Leavitt, a Professional Land Surveyor, licensed by the State of Wyoming, being fIrst
duly sworn on oath, do hereby depose and say:
1. That I am the Professional Land Surveyor responsible for the filing of said Plat.
2. That under the certificate of owners the access road is referred to as Cottonwood Lane, the
intended name, to replace this statement should say, Packstring Road.
3. That under the certificate of owners the owners Ivan S. Johns and Jean R. Johns hereby need
to sign and date with acknowledgement that said changes to the certificate of owners has been
made by the recording of this affidavit of correction.
