AFFIDAVIT TERMINATING ESTATE BY
JOINT TENANCY
I, Sherry L. Levenhagen, being of lawful age and first duly sworn accordi g to law,
upon my oath, depose and state:
1. That Lucille Wainwright died on Oecember 31, 2007 in Kemmerer,
2. That on January 27, 2005 for valuable consideration Lucille Wain right by
by her QuitClaim Deed of that date, which deed was duly filed for record in the Office of
the Lincoln County Clerk on January 27, 2005 in 577PR on page 868, conv yed unto
Lucille Wainwright and Sherry L. LeveLnhagen, as joint tenants with full rights of
survivorship, the following described real property, to wit:
Lots 57 and 58 of Commissary Ranch Unit C-l, Lincoln
described on the official plat thereof.

3. That by reason of the said conveyance, Lucille Wainwright and Sherry L.
Levenhagen became the owners of the real property as joint tenants and title ther to vested
in them continuously from said date of conveyance as described in said QuitCl im Oeed,
until the date of death of Lucille Wainwright on December 31, 2007 at which ti e title to
the above described real property vested absolutely in Sherry L. Levenhagen in a cordance
with the provisions of §2-9-1 02, W.S. (1980),
4. Affiant avers and certifies that deceased is the identical party na ed with
Affiant in the aforementioned deed whose death terminated her interest, title an estate in
the said real property; and Affiant attaches hereto and makes a part of this Affida it a copy
of the official certificate of death of decedent, duly certified by the public a thority in
which said death certificate is a matter of record.
