COME NOW A. VerDean Jensen and Vema R. Jensen, who, being duly sworn and
under oath, hereby state, pursuant to Wyoming Statutes § 34-2-123, as follows:
1. That the Warranty Deed that was recorded in the land records of Lincoln County,
Wyoming on September 6, 2002 in Book 498 PR at Pages 498 and 499 as Instrument No.
883823, a copy of which is attached hereto, failed to include the information that is required by
Wyoming Statutes § 34-2-122 to define properly our Trust and therefore, it failed to vest the title
to the property described therein in our Trust.
2. That the title to the lands described in the attached Warranty Deed were intended
and are now intended to be vested in A. VerDean Jensen and Vema R. Jensen, Trustees of the A.
VerDean and Vema R. Jensen Family Trust dated July 18,2002.

3. That we execute this Notice of Trust both as Settlors of, and as Trustees of, the A.
VerDean and Vema R. Jensen Family Trust dated July 18,2002.

hereby acknowledged, CONVEYS AND WARRANTS to the A. VERDEAN AND
VERNA R. JENSEN FAMILY TRUST, da~ed August 26'h ,2002, grantee of the County
of Lincoln, State of Wyoming, the following described real estate situated In County of
Lincoln and State of Wyoming, hereby releasing and waiving all rights under and by
