Affidavit of to the Public

I, David Podlesnik, being of lawful age and duly sworn according to law, upon
my oath, depose and state:
That under the date of January 26, 2000, for valuable consideration, Clara 1.
Podlesnik, a single person, by deed of that date, which deed was duly filed of record in
the Office of the Lincoln County Clerk, on January 28, 2000, in Book 440PR, Page 924,
conveyed to Clara 1. Podlesnik and David Podlesnik, as joint tenants, the following
described land, to-wit:
Lot 10 and the easterly 1 foot of Lot 9 of Block 43 of the First Addition to the Town of
Kemmerer, Lincoln County, Wyoming
That by reason of said conveyance aforesaid, the said Clara 1. Podlesnik and
David Podlesnik, became the owners of said real property, and title thereto vested in them
continuously from the date of said convèyance to the date of death of Clara 1. Podlesnik,
on the 22nd day of December, 2006. That by reason of and upon the death of Clara 1.
Podlesnik, title to the above described real property vested in David Podlesnik.
Affiant avers and certifies that Clara 1. Podlesnik is the identical party named
with David Podlesnik in the aforementioned deed, whose death terminated her interest,
title and estate in said real property; and Affiant attaches hereto and makes a part of this
affidavit, a copy of the Official Certificate of Death of said decedent, duly certified by the
public authority in which said death certificate is a matter of record.
