through its undersigned officer, does hereby make the following statement of facts:
1. That 1st Bank holds a Mortgage with Paul C. Jenkins and Sheila Jenkins, dated 29 April
2008 and recorded 5 May 2008 in the Office of the Clerk of Lincoln County in Book 693 of
Photostatic Records on page 653, all of the property described under the Certificate of
Surveyor on the plat titled, "STATELINE CONDOMINIUMS PHASE IV BUILDING 100
IDENTICAL WITH LOT 1 OF STATELINE ESTATES WITHIN GLO LOT 1
NW1/4NE1/4 SECTION 33 T35N Rl19W LINCOLN COUNTY, WYOMING", dated 21
August 2008, as revised.
2. That the undersigned officer has reviewed said plat.
3. That she has read and understands the provisions of the Certificate of Mortgagee thereon
and the implications thereof.
4. That this certificate is signed in the stead of and has the same effect as if the original mylar
of said plat had been signed.
