TO WHOM IT MAY CONCERN:
Notice is hereby given that the undersigned JEREMIAH FALSLEV, as Estimator of
Consolidated Paving and Concrete, Inc., with its principal place of business at 1705 West 2450
South, Ogden, Weber County, Utah, 84401, hereby claims and intends to hold and claim a lien
upon that certain land and premises, owned and reputed to be owned by ANTELOPE RIDGE
DEVELOPMENT, LLC, of 42 N. 200 East, American Fork, UT 84003 known as Antelope Ridge
Subdivision, Kemmerer, County of Lincoln, State of Wyoming, more particularly described as
follows, to-wit:
All of Lots of Antelope Ridge Subdivision, according to the official plat thereof as more
specifically set forth on "Attachment A" incorporated by reference and made a part
hereof.
to secure the payment of the sum of TWO HUNDRED ONE THOUSAND THREE HUNDRED
FORTY-EIGHT AND 40lhundredths ($201,348.40), owing to the undersigned for corporation
for labor, materials, and/or services in, on and about the premises on said land.
That the said indebtedness accrued and the undersigned provided the materials and
services to Antelope Ridge Development, LLC, as the general contractor at the job site of the
property as aforesaid, under written proposals dated June 11, 2008 and October 1, 2008, by the
terms of which proposal the undersigned did agree to provide the labor, materials and/or services
and the said owners and general contractor did agree to compensate the undersigned the amount
yet owed of ONE HUNDRED NINETY-NINE THOUSAND THIRTY-SEVEN AND
40lhundredths DOLLARS ($199,047.30), which the undersigned herein seeks and the
undersigned did provide labor, materials, equipment and services the first day August 18, 2008,
and did provide the last day work and services on the October 1, 2008, and on and between the
said dates provided the agreed to labor, materials, equipment and/or services plus interest and
legal fees the basis of which gives rise to the claim on the part of the undersigned for the total
sum TWO HUNDRED ONE THOUSAND THREE HUNDRED FORTY-EIGHT AND
40lhundredths ($201,348.40), which was the reasonable value for the services provided by the
undersigned, and all of which amount remains due and owing to the undersigned, and for which
demand the undersigned holds and claims a lien by virtue of Wyoming statutes 29-2-101 et seq.

in the foregoing Notice of Lien; and has the authority to execute said Notice of Lien on behalf of
the corporation; that he has read said Notice and knows the content thereof, and that the same is
true to his own knowledge.
