I, Kathryn R. Kulinski, being first duly sworn upon my oath depose and state as
follows:
1. That I am the Trustee named in the Kathryn R. Kulinski Family Trust dated
fYì(J I) 2008 .
2, That on May 30, 2008 in Book 695PR on page 818 of records of Lincoln
County Clerk was recorded a Quitclaim Deed from Kathryn R. Kulinski,
Trustee of the Kathryn R. Kulinski Living Trust dated September 26, 2001 to
Kathryn R. Kulinski, Trustee of the Kulinski Family Trust conveying the
following described land:
SEE ATTACHED EXHIBIT A
3. The Quitclaim Deed conveying the property to the trust was not properly
defined as required by W.S. 34-2-122 (1977) in that the date of the Trust was
not shown. The date of the Trust is h1rL'j [) 2008 .
