NOTICE OF MECHANIC'S LIEN
TO WHOM IT MAY CONCERN:

Notice is herby given that BIG CITY INSULATION doing business in the county of Washington
State of Utah, whose current address is 1140 SOUTH 1900 EAST #2 WASHINGTON UT 84780 and
whose current phone number is (435) 652-9106, hereby claims and intends to hold and claim lien for
$13,592.32 upon that certain land and premises in the LINCOLN County, State of WYOMING,
commonly know as 760 HIGHWAY 189-30, KEMMERER, WY 83101 and more specifically
described below. BIG CITY INSULATION further states as follows:
1. The record owner is BST HOSPITALITY LLC
2. Said indebtedness accrued and the undersigned provided MATERIAL AND
INSTALLATION OF INSULATION when SEVEN FOLD, LLC to provide
improvements to the property described herein, including but-not limited to providing
insulation materials and services under written and oral agreements by the terms of which
the undersign did agree to furnish insulation material and services, and SEVEN FOLD,
LLC did agree to pay the undersigned therefore.
The undersigned did provide the first labor and materials on 11/21/08 provide the last
labor and materials on 07/03/0$. The undersigned holds and claims a lien by virtue of the
provisions of Chapter 1, of Title 38, of the Utah Code Annotated 1953.
4. The real property subject to this mechanic's lien is situate, lying and being in the County
of Washington, State of Utah, the description of the property sufficient for identification,
described as follows, to wit:
Legal Description: BST HOSPITALITY ADDITION LOT 1
5. The amount claimed as a lien against the property is $13,592.32, plus interest, attorneys'
fees, and costs owing to the undersigned for insulation material and services which
benefited and improved said land.
6. PROTECTION AGAINST LIENS AND CIVIL ACTION. Notice is hereby provided
in accordance with Section 38-11-107 of the Utah Code that under Utah law an "owner"
may be protected against liens being maintained against an "owner-occupied residence"
and from other civil action being maintained to recover monies owed for "qualified
service" performed or provided by supplies and subcontractors as a part of this contract,
if and only if the following conditions are satisfied:
(1) the owner entered into a written contract an original contractor, a factory built
housing retailer, or a real estate developer;
(2) the original contractor was properly licensed or exempt from licensure under Title 58,
Chapter 55, Utah Construction Trades Licensing Act at the time contract was
executed; and
(3) the owner paid in full the original contractor, factory built housing retailer, or real
estate developer or their successors or assigns in accordance with the written contract
and any written or oral amendments to the contract
