That the name of the grantee in that Quitclaim Deed dated January 15, 1992, and recorded in the office of the Lincoln County Clerk on July 9, 1992, in Book 312 PR, page 441, Recording No. 751078, conveying the above-described real property is hereby stated to be 4 GOV08 0 as follows: Glenn W. Heiner, Trustee of the Glenn W. Heiner Family Revocable Trust dated January 15, 1992. 3. That her name and address is Rosann DeMill, 809 Corner Ridge Drive, Draper, UT 84020. 4. That she is the daughter of Glenn W. Heiner and Maxine Heiner and a beneficiary of the trust referred to, that Glenn W. Heiner became deceased on August 4, 1996, that Maxine Heiner became deceased on August 8, 2009, and that upon the death of Maxine Heiner, she became the successor trustee under the Glenn W. Heiner Family Revocable Trust dated January 15, 1992. 5. That as of August 8, 2009, the date of death of Maxine Heiner, Rosann DeMill became the successor trustee under the Glenn W. Heiner Family Revocable Trust dated January 15, 1992, including its ownership of the real property described above, and that she is currently lawfully serving in that capacity. 6. That she hereby verifies that the information contained in this affidavit is true and correct according to her information and belief. DATED as of the 16th day of September, 2009.
