provisions of W.S. 34 -2 -123 and in connection with the following described real property located in
Lincoln County, Wyoming:

The SE1 /4 of Section 23, T36N, R119W, 6th P.M., Lincoln County, Wyoming.
Parcel 2
Beginning at a point 80 rods West of the NE Corner of Section 27, T36N, R119W of
the 6th P.M., and proceeding West 80 rods, thence South 2 rods, thence East 80
rods, thence North 2 rods to the place of beginning, containing one acre, more or
Tess.
AND ALSO, Beginning at the NE Corner of Section 27, T36N, R119W of the 6th
P.M., Lincoln County, Wyoming, and proceeding West 80 rods, thence South 64°
East for 23.8 rods, thence East 69.6 rods, thence North 21.4 rods, to the point of
beginning, containing 10 acres, more or Tess.
AND ALSO, the S1/2SW1/4 of Section 22, T36N, R119W of the 6th P.M., Lincoln
County, Wyoming, containing 80 acres, more or Tess.
Parcel 3
SE1 /4SW1/4 of Section 23 and NW1 /4NW1/4 of Section 26, T36N, R119W of the
6th P.M., Lincoln County Wyoming, containing 80 acres, more ore Tess, together
with all improvements and water rights.

2. That the name of the grantee in that certain Quitclaim Deed dated November 10,
1994, and recorded in the office of the Lincoln County Clerk on January 4, 1995, in Book 362 PR,
page 803, conveying the above described real property is hereby stated to be as follows: Sybil A.
Crook, Trustee of the Sybil A. Crook Family Revocable Trust dated November 10, 1994 "the
Trust
3. That his name and address are Vern B. Crook, PO Box 5391, Etna, WY 83118.
4. That he is a surviving child of Sybil A. Crook and a beneficiary of the Trust; that Sybil
A. Crook became deceased on May 9, 2012; that upon the death of Sybil A. Crook, Vern B. Crook
became the sole trustee of the Sybil A. Crook Family Revocable Trust dated November 10, 1994.
5. That as of May 9, 2012, the date of death of Sybil A. Crook, Vern B. Crook became
the sole Trustee of the Sybil A. Crook Family Revocable Trust dated November 10, 1994. including
its ownership of the real property described above, and that he is currently lawfully serving in that
capacity.
6. That he hereby verifies that the information contained in this statement is true and
correct according to his information and belief.
