Phillips, husband and wife of the Pleasant Grove, in the state of Utah, do hereby certify that certain mortgage bearing date the 22 day of Oct, 2008, made and executed by Ann Wagstaff and Earl J. Blackham, as Joint Tenants with Rights of Survivorship, as mortgagor to David O. Phillips and Allyson B.
