THIS REASSIGNMENT of that certain Uniform Real Estate Lease (herein referred to as the "Contract is made and entered by and
between The Greater Salt Lake Business District of 2595 East 3300 South, Salt Lake City, Utah 84109 and The Small Business
Administration of 2237 Federal Building, 125 South State Street, Salt Lake City, Utah 84138 (hereinafter referred to jointly as "Assignor
for the benefit of Star Valley Wellness, LLC, a Wyoming limited liability company, of 124 Peterson Parkway, Thayne, Wyoming 83127
(hereinafter referred to as "Assignee
A. Assignee has heretofore entered into a Real Estate Lease "Contract dated October 29, 2004, affecting certain real property
located in Lincoln County, Wyoming, as is more particularly described in the attached Exhibit "A with Star Valley Wellness, LLC, a
Wyoming limited liability company, of 124 Peterson Parkway, Thayne, Wyoming 83127, as Lessor and Jim Wheeler Physical Therapy
Services, LLC, of 124 Peterson Parkway, Thayne, Wyoming 83127, as Lessee, as disclosed by certain Lease recorded February 22, 2005,
as Entry No. 906619 in Book 579 at Page 385 of the official records of the County Recorder of Lincoln County, Wyoming.
B. Assignee and Assignor have heretofore executed an Assignment of Real Estate Lease and Subordination Agreement as Collateral
dated September 24, 2004, recorded February 22, 2005 as Entry 906621 in Book 579 at Page 392 of the official records of Lincoln
County, Wyoming
C. Assignor now desires to terminate its assignee interest in the Contract and reassign its interest back to Assignee.
NOW, THEREFORE, in consideration of the premises Assignor hereby reassigns, transfers and conveys unto Assignee the Contract TO
HAVE AND TO HOLD the same for and during the remainder of the terms mentioned in the Contract and all renewals and extensions of
said term. Assignor does further terminate its interest in and to the Contract.

ALSO, a portion of that certain parcel of land referred to in the Deed
recorded in Book 379PR on page 643 with the Office of the Clerk of
Lincoln County, Wyoming, within the NWT /,SE4i of Section 23, T34N R119W of
the 6th P.M., located in the Town of Thayne,. Lincoln County, Wyoming
being more particularly described as follows:
BEGINNING at the Iron Pipe found marking a point in the Easterly line of
said parcel, said Point being 879.67 feet East to a Magnetic PK Nail set
at a point in the Center line of the Dana Street Right -of -Way and 40.39
feet S 7 °28'21" E from the Knowlton's B.L.M. type Monument marking the
Northwest corner of said NW
