COMES NOW M. KEVIN VOYLES, of LUTHI VOYLES, LLC, who, being duly sworn
and under oath, states as follows:
1. That the Warranty Deed from Thomas M. Baker, a single man, to Thomas M. Baker,
Trustee of The Thomas M. Baker Living Trust dated May 3, 1990, restated as amended by the Fourth
Amendment thereto dated May 28, 2013, recorded in the land records of Lincoln County, Wyoming on
June 6, 2013 in Book 813 at Page 110 as Receiving No. 971330 (copy attached), described the real
property conveyed therein as:
Lot 58 of Stewart Creek Country Club Estates Phase 2, Lincoln County, Wyoming, as
described on the plat thereof.
2. That the correct description of the property conveyed in said deed is:
Lot 58 of Stewart Country Club Estates Phase 2, Lincoln County, Wyoming, as described
on the plat thereof.
3. That the Warranty Deed from John M. Baker, Successor Trustee of The Thomas M.
Baker Living Trust dated May 3, 1990, restated as amended by the Fourth Amendment thereto dated May
28, 2013, to John R. Baker, a married man, recorded in the land records of Lincoln County, Wyoming on
March 28, 2014 in Book 829 at Page 590 as Receiving No. 975732 (copy attached), described the real
property conveyed therein as:
Lot 58 of Stewart Creek Country Club Estates Phase 2, Lincoln County, Wyoming, as
described on the plat thereof.
4. That the correct description of the property conveyed in said deed is:
Lot 58 of Stewart Country Club Estates Phase 2, Lincoln County, Wyoming, as described
on the plat thereof.
5. That the succession of John R. Baker as Successor Trustee of The Thomas M. Baker
Living Trust dated May 3, 1990, restated as amended by the Fourth Amendment thereto dated. May 28,
2013, is of record, pursuant to the Affidavit of Trust recorded in the land records of Lincoln County,
Wyoming on March 28, 2014 in Book 829 at Page 589 as Receiving No. 975731 (copy attached).
6. That John R. Baker, a married man, is the owner of Lot 58 of Stewart Country Club
Estates Phase 2, Lincoln County, Wyoming, as described on the plat thereof.
7. That this Affidavit is made pursuant to and in compliance with the provisions of
Wyoming Statutes 34 -11 -101.

valuable consideration, in hand paid, receipt of which is hereby acknowledged, CONVEYS AND WARRANTS to
THOMAS M. BAKER, TRUSTEE of the Thomas Baker Living Trust, dated May 3, 1990, as amended, P.O. Box
927, Thayne, Wyoming 83127, GRANTEE, and his successors and assigns, real property in Thayne, Lincoln
County, Wyoming, more particularly described as follows, to -wit:
Lot 58 of Stewart Creek Country Club Estates Phase 2, Lincoln County, Wyoming, as described
on the official plat thereof.
Together with all improvements, and appurtenances thereon situate or in anywise pertaining
thereunto.
Subject, however, to all covenants, conditions, reservations, restrictions, easements, and rights -of-
way of record, of sight, or in use.

Amendment thereto dated May 28, 2013, continues to exist, and has not been revoked, modified, or amended in any
manner that would cause the representations in this Affidavit to be incorrect.
SETTLOR OF THE TRUST: Thomas M. Baker.
CURRENT TRUSTEE: John R. Baker.

STATUS OF TRUST: This Trust is Irrevocable.
