COUNTY OF LINCOLN
PROFILE PROPERTIES, LLC, a Wyoming Limited Liability Company, acting through its manager, Scot
Cook, who being duly sworn on oath deposes and states as follows:
1. Profile Properties, LLC is the owner of Lots 80 and 81 of the Alpine Village Subdivision No. 1
Plat 2 Amended Fifth Filing of record in the Office of the Clerk of Lincoln County in Book 835
of Photostatic Records on page 871.
2. That Lots 80 and 81 of said Alpine Village Subdivision No. 1 Plat 2 Amended Fifth Filing were
created from a division of Lot 51 of Alpine Village Subdivision No. 1 Plat 2 Amended.
3. That the Alpine Village Realty, Inc. Declaration of Covenants, Conditions and Restrictions
recorded in said Office in Book 108 of Photostatic Records on page 115 designated the use of
said Lot 51 as commercial.
4. That the plat for Alpine Village Subdivision No. 1 Plat 2 Amended Fifth Filing was prepared by
Lloyd B. Baker and Associates, LLC and recorded in said Office with Accession No. 884518.
5. That the Land Use Table as depicted on the plat designates the type of use of the lots as
residential.
6. That the affiant requests the Land Use Table be amended to state Residential /Commercial.
7. This Affidavit is being filed pursuant to W.S. 34 -11 -101.
