COMES NOW Wayne M. Johnson whose address is 521 Cottonwood Drive, Cheyenne,
Wyoming 82001 who, being duly sworn on oath according to law, does hereby make the
following statements of facts and affirm:
That he is the owner of the land described under the Certificate of Surveyor on the
plat titled, "JOHNSON FAMILY ESTATES WITHIN GLO LOT 4 (NE¼NE¼)
SECTION 22 T31N R119W LINCOLN COUNTY, WYOMING," dated 22 January
2003, as revised.
2. That he has reviewed said plat.
3. That he has read and understands the provisions of the Certificate of Owner thereon
and the implications thereof.
4. That this affidavit is signed in the stead of and has the same effect as if the original
mylar of said plat had been signed.
