COMES NOW, G & M Properties II, L.C., a Utah Corporation acting through its
Manager, Gregg H. Wilding, whose address is 572 South 300 East, Brigham City, Utah
84302 who, being duly sworn on oath according to law, does hereby make the following
statements of facts and affirm:
That G & M Properties II, L.C., a Utah Corporation isthe owner of Lot 216, Lot 217,
and Lot 218 as depicted on that plat titled, "LAKE VIEW ESTATES
INCORPORATED TRACT (A) A SUBDIVISION BEING A PART OF S½ OF
SE¼ SEC. 29, T37N, R118W, ALPINE, WYOMING" of record in the Office of the
Clerk of Lincoln County, Wyoming as Plat No. 158.

That the undersigned has reviewed the plat titled, "LAKEVIEW ESTATES THIRD
ADDITION BEING PART OF LAKEVIEW ESTATES INCORPORATED TRACT
(A) WITHIN THE INCORPORATED LIMITS OF THE TOWN OF ALPINE
WITHIN S½SE¼ SECTION 29 T37N R118W LINCOLN COUNTY, WYOMING",
dated 15 October 2001, as revised.
3. That the undersigned has read .and understands the provisions of the Certificate of
Owner thereon and the implications thereof.
4. That this affidavit is signed in the stead of and has the same effect as if the original
mylar of said plat had been signed.
