I, ROBERT F. SORENSEN, being duly swom under oath, state as follows:
1. Carolyn J. Sorenson has joint tenancy with me in land in Lincoln County,
Wyoming, more particularly described as follows:
Lot 23 in Star Valley Ranch Plat 12, Lincoln County, Wyoming, as described on
the official plat thereof recorded in the Office of the County Clerk, Lincoln County,
Wyoming.
Subject to reservations and restrictions contmned in the United States Patent and
to easements and rights-of-way of record or in use.
Together with all improvements and appurtenances thereon.
Attached hereto is a copy of the Warranty Deed creating the joint tenancy, recorded in the
Lincoln County, Wyoming, land records in Book 298PR tit Page 378, on July 9, 1991 as
Instrument No. 735041.
2. I, Robert F. Sorensen, the surviving spouse of Carolyn J. Sorensen, hereby certify
that Carolyn J. Sorensen died on February 7, 2003. Attached hereto is an original copy of the
Certificate of Death issued by the State of Idaho for Carolyn J. Sorensen.
3. Pursuant to Wyoming Statutes § 2-9-102, I certify that the tenancy of Carolyn J.
Sorensen has been terminated by her death, and that title to the above-referenced land is now in
the name of Robert F. Sorensen, a single man.
