IJ ttle Ave Driggs, Idaho, Makes the following statement and claim of lien pursuanl to section 29-1-2-1, et .s. eq Wyoming Statutes, 1977, as amended: 1. Stock Building Supply (hereinafter referred to its "Claimant") furnish Building materials and supplies for use in the permanent improvement of the Following described real property located in the state of \Vyoming: Lot 38 of the Star Valley ranch Plat 14, Lincoln ('ounty, Wyoming as described on the official plat thereof. o The name and address of the owner (s) or relm~,:d owner of said real property, at the time this statement and claim of lien was made, is as follows' NOE PEREZ GARCIA 558 ALTA DRIVE TI IA YNE, WY 83127 Claimant furnished said building materials and Suloplies at the specific instance and request of Guerdon Village Vallimla ~INC. Who was the acting for the use and benefit of the owner(s) or reputed owner(s) of said real property, under the terms of an open account agreement more particularly, identified as follows: A description of the building materials and SUl,plics furnished by claimant for use in the improvement of said real property is set forth in exhibit "A" attached hereof. Claimant commenced furnishing said buildin,.2, materials and supplies on August-17th 2004, and last delivered building~malcrials and supplies for use in the improvement of said real property on Seple,nbe,- 8th 2004. Claimant has not received full payment for said building materials and supplies. Despite repeated request therefore, and there is now due, owing and unpaid claimant, including finance charges con]puted through the last billing date prior to the within claim of lien, and after allowing all just credits and off-sets, the sum of $ 8107.73. Said building materials and supplies were incorporated into and contributed to the permanent improvement of said real propert v, and benefited the owners(s) or reputed owner(s) thereof. Claimant gave a timely notice of its intention to file a claim against said real property and improvements, in accordance with section 29-2-107 of the Wyoming Statutes, 1977, as amended, a copy of which is attached as Exhibit "B" hereto and by this reference mane a part hereof. 9. A copy of the within statement and claim of lien shall be sent, by certified mail, to the last know owner(s) or reputed Owner(s) of said real property promptly after it is filed with the county clerk of the county where said property is located.
