BALLOON LOAN MODIFICATION
TillS LOAN IS PAYABLE IN FULL AT MATURITY. YOU MUST REPAY THE ENTIRE PRINCIPAL
BALANCE OF THE LOAN AND UNPAID INTEREST THEN DUE. THE LENDER IS UNDER NO
OBLIGATION TO REFINANCE THE LOAN AT THAT TIME. YOU WILL, THEREFORE, BE
REQUIRED TO MAKE PAYMENT OUT OF OTHER ASSETS THAT YOU MAY O~rN, OR YOU
WILL HAVE TO FIND A LENDER, WHICH MAY BE THE LENDER YOU HAVE THIS LOAN WITH,
WILLING TO LEND YOU THE MONEY. IF YOU REFINANCE THIS LOAN AT MATURITY, YOU

THIS MODIFICATION IS TO BE EXECUTED IN DUPLICATE ORIGINALS: ONE ORIGINAL IS TO
BE FILED WITH THE BALLOON NOTE AND ONE ORIGINAL IS TO BE RECOILDED IN THE
LAND RECORDS WHERE THE SECURITY INSTRUMENT IS RECORDED
This Balloon Loan Modification ("Modification"), made November 30, 2004, between Shelley Irene Berry
("Borrower"), and JPMorgan Chase Bank, N.A. ("Lender"), amends and supplements (1) the Mortgage, Deed of
Trust or Deed to Secure Debt (the "Security Instrument") dated September 9, 2002, securing the original
principal suni of U.S. $32,300.00, and recorded on September 12, 2002, in Document No. 884015, Book 49.~8.
Page 871, in the Official Records of Lincoln County, Wyoming and (2) the Balloon Note bearing the same date
as, and secured by, the Security Instrument, (the "Note") which covers the real and personal property described in
the Security Instrument and defined in the- Security Instrument as the "Property", located at Off of Lincoln
County Road 123, Bedford, Wyoming 83112, the real property described being set forth as follows:
A TRACT OF LAND IN THE NW 1/4 SW 1/4 OF SECTION 11, T33N Rll8W OF THE 6TH P.M.,
LINCOLN COUNTY, WYOMING BEING MORE PARTICULARLY DESCRIBED AS FOLLOWS:
BEGINNING AT THE NORTHWEST CORNER OF TIlE NW1/4SW1/4 OF THE SAID SECTION 11,
THENCE SOUTH 16 RODS, THENCE EAST 20 RODS, THENCE NORTH 16 RODS, TItENCE WEST
20 RODS TO THE POINT OF BEGINNING.

To evidence the election by the Borrower of the Conditional Modification and Extension of Loan Terms to
modify the terms of the Note and Security Instrument in accordance with such election, Borrower and Lender
agree as follows (notwithstanding anything to the contrary contained in the Note or Security Instrument):
The Borrower is the owner of the Property.
As of October 1, 2004, the amount payable under the Note and Security Instrument (the "Unpaid
Principal Balance") is U.S. $29,606.24.
The Borrower promises to pay the Unpaid Principal Balance, plus interest, to the order of the Lender.
Interest will be charged on the Unpaid Principal Balance at the yearly rate of 6.00%, beginning
October 1, 2004.
The Borrower will make payments every month beginning on November 1, 2004. The Borrower will
continue to make payments on the same day of each succeeding month until principal and interest are paid
in full. The monthly payments will be applied to interest before principal. If on October 1, 2005 (the
"MOdified Maturity Date"), the Borrower still owes amounts under the Note and the Security Instrument,
as amended by this modification, the Borrower will pay these amounts in full on the Modified Maturity
Date.
The monthly payment to be paid by the Borrower will be in the amount of U.S. $148.03.
The Borrower will make such payments at 3415 Vision Drive, Columbus, Ohio 43219-6009 or at
such other place as the Lender may require.
The Borrower has the fight to make payments of principal at any time before they are due. A payment of
principal only is known as a "Prepayment". When the Borrower makes a Prepayment, the Borrower will
inform the Note Holder in writing that they are doing so.
The Borrower may make a Prepayment without paying any Prepayment charge. The Note Holder will use
all of the Prepayments to reduce the amount of principal that the Borrower owes under the Note as
amended by this modification agreement. If the Borrower makes a partial Prepayment, there will be no
changes in the due date or in the amount of my monthly payment unless the Note Holder agrees in writing
to those changes.
The Borrower will comply with all other covenants, agreements, and requirements of the Note and the
Security Instrument, including without limitation, the Borrower's covenants and agreements to make all
payments of taxes, insurance premiums, assessments, escrow items, impounds, and all other payments that
the Borrower is obligated to make under the Security Instrument; as of the maturity date of the Note.
Nothing in this Modification shall be understood or construed to be a satisfaction or release in whole or in
part of the Note and Security instrument. Except as otherwise specifically provided in this Modification,
the Note and Security Instrument will remain unchanged and in full effect, and the Borrower and Lender
will be bound by, and will comply with, all of the terms and provisions thereof, as amended by this
Modification.
